Application_for_Certified_Copy --Please bring this form in person to the Municipal Clerk's Office at 770 Coopertown Rd or the State Registrar's office in Trenton--Only authorized persons may obtain certified copies of vital records. Photo I.D. required to obtain certified copies of vital records. Call 856-461-0561 ext 255 for more information.
